Your organisation relies on your expertise as an event investigator to identify the causes of events and suggest effective interventions to reduce the likehood of recurrence. The pivotal part of a Human in the System Investigation is the interview. The ability of the interviewer to explore the interviewee’s experiences often unlocks the key learning points from an incident.
This course builds on your existing knowledge gained through previous training and your hands-on experience in the workplace. It provides the opportunity to fine-tune your investigative interviewing skills and identify effective and sustainable interventions.

Designed for event investigators that wish to refresh their interview techniques in a supportive environment that builds confidence and explores the latest interview technique thinking, this two-day course will expand your error management capabilities and increase your contribution to improving your organisation’s safety performance.

  • Improve your ability to determine and map out areas to explore during your investigation
  • Improve interview planning so that you achieve more consistent outcomes
  • Develop greater confidence in your interview techniques through practical experience, observation and peer review
  • Enhance your ability to assess the data gathered and present it in a consistent manner to support organisational learning
  • Identify and apply effective and sustainable interventions to enhance safety

  • A brief revision of Human Factors and Error Management principles
  • How to effectively plan and manage the ongoing investigation
  • Improvement of interview techniques such as ‘timeline/funnelling’ and investigation protocol
  • Extensive ‘investigation scenario’ practical exercise
  • Analysis and presentation of evidence and results
  • Identification of effective interventions to reduce the likelihood of reoccurrence
 
Is this course right for me?
This Practitioner level course is primarily designed for previously trained investigators who wish to develop or refresh their skills. It is particularly suitable for previously trained investigators who have not had the opportunity to practise their skills in live investigations.
It is appropriate for Human Factors, Quality and Safety Managers and Safety Management Coordinators.

This course would also benefit
  • Regulatory Authority Inspecting Staff
  • Quality Assurance Staff who carry out internal quality investigations
 
Prerequisites
You will be expected to have attended a previous Investigator training course and have had exposure to event investigations, either as part of an investigation team or observer. Delegates should have completed a Human Factors course prior to attending this course.
 
Course details
Course format: This course is a balanced mix of group discussions, flipchart work and PowerPoint presentations. Managed syndicate exercises give you the opportunity to practise the application and evaluation of new processes and techniques.
Course level: Practioner Level Training (P) is suitable for those with relevant experience in the subject matter who are looking to enhance their specialist skills and knowledge and keep abreast of latest technical developments. Practioner level training also applies to Recurrent and Refresher training programmes as well as to courses that we recommend are taken in a chronological order.
Assessment process: No formal assessment. Formative feedback is given by the facilitator throughout.
Course size: 8 persons maximum at our Aviation Safety Academy or on-site at your facility
